چکیده

Background Cardiac resynchronization therapy (CRT) benefits selected heart failure patients; however, CRT has a nonresponse rate of approximately 30-40%. Important factors that contribute to lead placement decisions and overall CRT success include determination of mechanical dyssynchrony, presence and location of scar, and the identification of late-activated segments. Recent imaging studies have reported progress in the first two areas. Previously, myocardial tagging showed promise for mapping mechanical activation time [1], but analysis of tagged images is time consuming. We investigated the use of cine DENSE to detect regions of late mechanical activation.

Cine DENSE MRI of mechanical activation in heart failure patients referred for cardiac resynchronization therapy

Background In patients with heart failure (HF) and left bundle branch block (LBBB) cardiac resynchronization therapy (CRT) improves left ventricular (LV) function and leads to reverse LV remodeling. A major concern with CRT remains that using current clinical indicators and implementation methods, 30-40% of patients are nonresponders. CRT response is highly dependent on the presence of delayed ...

Cardiac mechanical activation mapping in heart failure patients with left bundle branch block using cine DENSE MRI

Background Cardiac resynchronization therapy (CRT) is an effective treatment for selected patients with heart failure (HF) and left bundle branch block (LBBB). However, an ongoing major issue with CRT is that 30-50% of treated patients are non-responders. One potential cause of a poor response is implantation of the CRT left-ventricular (LV) pacing lead at a suboptimal location, i.e., a locatio...

Comparison of heart deformation analysis and cine DENSE in volunteers and heart failure patients

Background Post-processing methods have been developed to quantify myocardial strain from steady-state free-precession (SSFP) cine images. In the left ventricle (LV), global strain measurements from SSFP in healthy volunteers and in some types of heart disease compare favorably with myocardial tagging as a reference. We compared heart deformation analysis (HDA) of SSFP images and cine Displacem...

Cardiac resynchronization therapy in an elderly patient with left and right ventricular noncompaction.

We present a case involving a 67‐year‐old man with newly diagnosed heart failure and left bundle branch block. Echocardiography revealed the pri‐ mary cause of heart failure, namely, ventricu‐ lar noncompaction, confirmed by cardiac mag‐ netic resonance imaging (MRI). Due to ventric‐ ular dyssynchrony, a cardiac resynchronization system was implanted. It was a rare etiology of heart failure tha...

Left Bundle Branch Block

Coronary angiography, cine left ventriculography, and the cardiovascular hemodynamics of 24 patients with left bundle branch block were studied. Seven patients were found to have significant coronary artery disease, always severe. Five had cardiomyopathy and seven had valvular disease. Only those with coronary artery disease or cardiomyopathy had abnormal left ventricular contraction patterns. ...
